Is it really a one-time payment?

Will it change my WHMCS files?

No. It installs as a normal addon module.

Does it work with all my products?

You choose. Tick the products that should offer renewal options, or turn it on for all products.

Does the Auto-Renew switch change how WHMCS creates invoices?

No. It records the client's choice for each service. It does not change the way WHMCS itself generates invoices.

Will my customers lose anything if there is a license problem?

No. If the license becomes invalid, the options stop showing, but anything a client has already paid for is still applied.
